Bias Incident Report

This form, to the Community Bias Response Team (CBRT), is for reporting bias incidents that occur on campus or in the local community. Bias incidents are those where speech or behavior is directed toward an individual or group and is based on or motivated by the individual or group’s real or perceived age, race, ethnicity, sex, gender identity, gender expression, sexual orientation, religion, disability status, national origin/nationality, citizenship status, language, veteran status, socioeconomic status, pregnancy status, body size, or political affiliation.

In addition, please utilize the Title IX online reporting form (https://go.middlebury/report) to report incidents of sex discrimination or harassment that occur on campus or in the local community. This includes sexual assault, sexual misconduct, domestic or dating violence or misconduct, and stalking. Middlebury encourages individuals to report these incidents. Reporting allows for easier and faster access to services and support to those affected, as well as allows Middlebury to respond.

All bias incident reports submitted using this form will be confidential to the extent permitted by law. If a bias incident alleges criminal or illegal conduct or suggests an ongoing threat to campus safety, CBRT is required to forward the report to Public Safety.
